The design questionnaire is malfunctioning again. Trying to figure out why now. Don't you just love when Google, in this case Google Docs, hates you? If you fill it out and it won't submit or if you're not sure if it went through, please email me at bloggingbelladesigns@yahoo.com. We'll just do an order that way.
And it may be a browser thing, by the way. I've only checked it in Firefox 4. I will let you know what I find out.
